Patients who use Philip’s Motiva system to help them better manage their long term condition/s such as diabetes, heart or breathing problems will be asked to complete a questionnaire. These provide useful feedback but also play an educational role providing information for example on lifestyle matters such as diet.The questionnaires are somewhat limited as the response is limited to numbers.
